Patient Access is available for you to request repeat prescriptions, book appointments or view your medical record on the internet.  

Being able to view your record online could help you to manage your medical conditions.  It also means that you have access from anywhere in the world should you require medical treatment whilst on holiday.  The service is available at all times and can be accessed from any computer, tablet or mobile phone.

To sign up you will need to complete one of the forms below.  The proxy form is for anybody over the age of 13 who wishes to nominate someone else to manage their account, such as next of kin or carer. Please read the information leaflet and complete the relevant form and bring it to the surgery with two forms of identification, one to be photographic such as driving licence or passport, and the other to be proof of address such as a utility bill.  The practice routinely writes to children aged 13 asking them to either reconsent to manage their record themselves or for their parent to manage their record on their behalf.
